Before You Ship – Download Form
- Sterilize each handpiece to be serviced and leave in sterilization pouch
- Fill out handpiece repair form complete as much as possible and make a copy for your records. Mail one copy with your handpieces
- Pack in an appropriate box with copy of repair form
- Attach the pre-paid mailing label
- Send package and your repair is on its way!
